Chronic Condition Special Needs Plans (C-SNP).
A Chronic Condition Special Needs Plan (C-SNP) is a type of Medicare Advantage plan limited to beneficiaries who have specific chronic conditions defined by Medicare. C-SNPs are designed to coordinate care, prescriptions, and provider networks around those conditions.
Who qualifies for a C-SNP
Eligibility is condition-based. CMS approves specific chronic conditions for C-SNP enrollment, including (but not limited to):
- Cardiovascular disorders
- Chronic heart failure
- Diabetes mellitus
- End-stage liver disease
- End-stage renal disease (ESRD)
- Severe hematologic disorders
- HIV/AIDS
- Chronic lung disorders
- Dementia
- Stroke
Carriers offering C-SNPs may not offer plans for every condition. Available conditions and carriers vary by service area.
How C-SNPs are different from a standard Medicare Advantage plan
C-SNPs include the same Part A and Part B coverage as Original Medicare, plus Part D drug coverage, plus benefits and care coordination tailored to the qualifying condition. Plan structure, networks, and formularies are designed with the chronic condition in mind.
How to enroll
C-SNP enrollment requires verification of the qualifying condition (typically a physician attestation). Special Enrollment Periods may apply when you become eligible for a C-SNP.
How a licensed agent helps
A licensed El-Mag agent helps verify C-SNP eligibility for your specific condition, compares C-SNP options against standard Medicare Advantage and Original Medicare + Medigap, and walks through enrollment.
